If your tenant applied for Allegheny County Emergency Rental Assistance (ERAP) and you’d like to participate, please:

Download and complete the following forms:

  • W9 completed and signed within the last year (no earlier than 2020)
  • Rent Amounts Owed to show a monthly breakdown of what the tenant owes including rent, utilities and fees like late, pet or parking fees. These fees must be reflected in a signed lease.
  • Authorization Agreement for Automatic Entries and a voided check.

Scan in or take a photo of a signed copy of the full lease showing rent and fees.
